This is a US Navy Hornet from Test and Evaluation Squadron VX-4 based at NAS Pt Mugu, CA. This mishap occured in 1981-82 timeframe during the aircraft's initial operational evaluation (OPEVAL). The the right main tire fell off the jet during takeoff. The pilot suffered a few broken bones during his parachute landing fall as a result of fairly high surface winds.

Unfortunately the video isn't of great quality so it's impossible to see if the F-18 features both the painted false canopy and the identification light on the fuselage forward right side! It may be either a Canadian or US Hornet so if anyone has more info... :-)
As for the ejection, I think the pilot feared that the aircraft could have cartwheeled; I can't blame him at all, happy he's ok! ;-)
